Better than nothing. How do I do it?

Sorry...life. 

I remember from a lot of my other help asking you've had me remove noise and it always seems overkill to me how far you want me to take it. So I'm curious what the rule is. The luminance on these is set to 25 but the overall image looks too smooth to me. 

I would love any advice on how to edit out these issues.

So I had my husband take a picture of my hand in the same position. Haha! That's my finger in the middle there. 

I've got this little pickle to fix in a few images. Please help!

I did burn at 10% exposure on the mid tones.  

And I'm using the burn tool, right? 

Am I changing the blank layer to soft light? Because nothing happens when I paint.

Is this right? Nothing happens when I dodge or burn. 

Much! Is there anything else I need to do?

I swear I followed that tutorial exactly! I'm sorry, I'm just not getting this.

Yes, I've tried that, full opacity, low opacity, doesn't matter. I'm still getting the same result. I don't understand how you can do it so smoothly. 

I'm confused. You said to use a brush as big as two baby toes, which is what I did.
