Masking

I have photoshop elements 13.  When masking, I am unable to get a clean 100% mask.  It always leaves just a hint of the layer.  The same thing happens if I am correcting a mask with a white brush at 100%.  I am not able to completely uncover the mask.  Any ideas why this would be happening?

Have you pressed D to make sure your swatches are pure black and pure white?

Yes, when I get close to 100% covering, it begins to kind of flicker.  When I am using white it leaves the faintest gray.

Here is a screen shot of a layer.  I used the select tool to select an area and then clicked on the area with a 100% white paint brush to show how it leaves a light gray.
