Solid Colour Issue

Happening to another image, using mouse or pen.  when I brush away it wont brush back 100%.  the layer is 100% solid to start with

Yeah, the mask definitely isn't white.

When you Alt-click on the mask thumbnail to view in black and white, then run your mouse over those areas and watch the Info Panel, you'll see it's kinda 2% and 3%.

So again I ask.  Did you press D to reset the swatches to white on black?

Yes I did press D, even twice after you mentioned it.

And then if you click on the white swatch to bring up the Color Picker dialog, is it saying 255/255/255 white?

The numbers are 255 255 255.  I have been having issues with my PC for a little while now, I seriously think it is time for it to go to retirement.

Is your clone tool exhibiting the same behaviour?

Not the exact behaviour as I do clone on a low opacity but where I select the part of the image to be cloned with the ALT key and I start to clone the cross hair that it is cloning from is no where near the selection it can come from, it randomly selects any where near the cloning circle.
