For myself, as needed. I don't think I have ever sent my camera in for a "cleaning." I have a rocket blower and blow out the sensor before a gig. I also use lens wipes, a lens pen, etc. If you are looking for a "Wet Cleaning," I recommend Nikon or Canon's service Dept. or even check out KEH.com's service dept. I don't recommend doing a direct sensor cleaning or "wet cleaning," too risky. If you screw up, it's a new camera body. That said, lots of people clean their own sensors with no problems. I'm not one of them.

I sent my camera out to be cleaned once and it came back dirtier so now I do it myself. I watched a video on KelbyOne a long time ago on how to clean the outside and inside of a camera and I only clean the sensor when there are visible spots so I do it maybe once a year.

I know it's risky, but I do it anyway since I don't know of a better way to get the job done and rocket blowers don't get the spots off. I clean the outside of the camera and my lenses much more often, usually before every shoot or every couple of weeks if I'm shooting a lot.

I'd recommend these Cotton Swabs for Electronics. They aren't as "fuzzy" and have a wooden stem, so they stay rigid.

Wading in late, but I get my wet cleaning stuff here

I've never had a problem; 8 different Nikon bodies of my own and several (even Canons!) for friends.

First course of attack is ALWAYS blowing with a Giottos Rocket blower, then going wet if absolutely required. Make sure that your battery is fully charged and it's not a big deal.

How often? Whenever it needs doing. Minimise lens changes in dusty areas, blowing it out with the Rocket regularly, and trying to keep the whole lot out of air conditioning to minimise condensation, which sticks dust to sensors and lens elements (A/Cis very bad for humans, too) seems to work for me.
