This is indeed a quandary. Never be afraid to leave a bit of noise in an old photo, if it also means you can preserve detail. Old photos are the opposite to digital photos in this regard.

Then press Ctrl Alt Shift E to create a new layer above. On that layer, go to Filter>Noise>Reduce Noise. Apply it to your taste. Then start the patient cloning repairs.

The method to my madness is about to become clear, I hope. Zoom in to 100% view, then go to your Channels panel. Click on the word "Red", then "Green", then "Blue". Compare them I think you'll agree that the red channel has the nicest quality detail.
